Whenever a pilot is prescribed or stops using either a new dosage of a medication or a new medication, the pilot must submit that information within 10 days to the physician appointed by the Board of Pilot Commissioners who has the pilot's prescribed medication list (that was previously submitted to the same Board of Pilot Commissioners appointed physician for the pilot's physical examination determining fitness for duty). Whenever the physician receives the updated information, the physician must determine whether the medication change affects the pilot's fitness for duty.
A provider of health care or a health care service plan may disclose medical information in the following circumstances: 1) to other healthcare providers to facilitate diagnosis and treatment 2) to find financially liable party and obtain payment 3) to administrative subcontractors 4) quality control organizations (peer review boards, etc.) 5) accrediting agencies 6) coroners 7) \bona fide research purposes\" 8) employer
The Veterinary Medical Board has the authority to place a licensee or registrant on probation. The board may require the licensee or registrant to submit to a complete diagnostic examination by board-appointed physicians. Accordingly, if the board requires a licensee or registrant to submit to that examination, the board shall receive and consider any other report of a complete diagnostic examination given by physicians chosen by the licensee or registrant.
In reinstating a license or registration that has been revoked or suspended, the Veterinary Medical Board may impose terms and conditions to be followed by the licensee or registrant after the license or registration has been reinstated. This includes requiring the licensee or registrant to submit to a complete diagnostic examination by board-appointed physicians. If the board requires the licensee or registrant to submit to that examination, the board shall receive and consider any other report of a complete diagnostic examination given by physicians chosen by the licensee or registrant.
After a committee in its discretion has determined that a dentist licentiate has been rehabilitated and the diversion program is completed, the committee shall purge and destroy all records pertaining to the licentiate's participation in a diversion program. Treatment records are confidential.
Boxing, wrestling, martial arts commission shall make available contestant's medical record to physician for certification of physical condition. Report of medical exams shall be filed with commission (physician's report shall include specific mention as to the condition of the contestant's heart, nerves and brain).
Any medical records obtained or created for the purpose of licensure of professional athletes may not be disclosed by the licensing commission except under certain circumstances, as provided. If, after a process for participation in medical research has been adopted, the athlete consents to participation, the medical records may be used by the commission for medical research. The medical information shall not include any personally identifiable information.
Unprofessional conduct by licensed dental hygienist includes the failure to report to the Dental Hygiene Committee in writing within seven days: (1) the death of his or her patient during the performance of any dental hygiene procedure; (2) the discovery of the death of a patient whose death is related to a dental hygiene procedure performed by him or her; or (3) except for a scheduled hospitalization, the removal to a hospital or emergency center for medical treatment for a period exceeding 24 hours of any patient as a result of dental or dental hygiene treatment.
A registered dental hygienist shall report to the Dental Hygiene Committee all deaths occurring in his or her practice with a copy sent to the Dental Board if the death occurred while working as an employee in a dental office. A dentist shall report to the dental board all deaths occurring in his or her practice with a copy sent to the committee if the death was the result of treatment by a registered dental hygienist.
